Name Change or Corrected Social Security Card

How to update the name on your Social Security card after marriage, divorce, or a legal name change — and how to correct other information.

The short answer

If you need to change your name or correct information on your card, you cannot use the online replacement service. You'll apply by mail or in person with Form SS-5, your legal proof of the name change, and a current identity document. The new card has the same number, just with the updated name.

When you need a new card

  • Your name changed (marriage, divorce, court order, or naturalization).
  • The name on your card no longer matches the name you use on tax, banking, or employment records.
  • Other identifying information on the card is incorrect.

You don't need a new card every time you start a new job — your number is what matters.

Documents you'll need

  • Proof of legal name change: Marriage document, divorce decree, court order, or Certificate of Naturalization in the new name.
  • Proof of identity: A current, unexpired U.S. driver's license, state ID, or U.S. passport.
  • Possibly an ID in your prior name: If the name change was more than 2 years ago (4 years if under 18), SSA may also ask for an identity document in your old name. It may be expired.

If you don't have an ID in your prior name, SSA can sometimes accept an unexpired ID in the new name if your identity can be verified in their records. Full list: documents needed.

How to apply

  1. Complete Form SS-5

    Download it at ssa.gov/forms/ss-5.pdf. Use your current legal name.

  2. Gather originals or certified copies

    SSA does not accept photocopies or notarized copies. Documents are returned after they're reviewed.

  3. Bring or mail to your local SSA office

    Find your office at ssa.gov/locator.

  4. Receive your updated card

    The card is free. Your Social Security number does not change.

Other updates after a name change

  • Notify your employer so your W-2 matches the SSA record.
  • Update your driver's license or state ID.
  • Update your passport, banks, insurance, and benefits accounts.
